The following applies to applications made within three years from the date of suspension

The following general reinstatement guidelines must be met in order for an applicant to be eligible for reinstatement. These include submission of the following:

  1. A completed reinstatement application form
  2. Payment of the current registration and reinstatement application fee. Section 4.1 (g) of the Fees By-Law, states: "The reinstatement fee for the reinstatement of a certificate of registration… is $125.00 [plus GST] for each year, or part of each year from the time that the certificate was suspended, up to a maximum of $1,250.00[plus GST]"
  3. A completed COBI or a completed Undertaking form, if not dispensing.
  4. Two colour, passport sized photographs signed on the reverse
  5. Evidence of having completed the requirements of the College’s Quality Assurance Program (thirty (30) credits for every cycle that you have missed while your Certificate of Registration was suspended)

The reinstatement application will be submitted to the Registrar for review after it has been processed for final approval.

Final Approval

Only upon the final approval of the application for reinstatement by the Registrar will the applicant have the legal right to dispense prescription eye glasses, contact lenses and sub-normal vision devices.
